Form for exercising the rights of data subjects

The different categories of Data Subjects (natural persons to whom the personal data are connected) which refer to the companies of the Lutech Group present on this website, may send their requests to exercise the rights by way of this form.

Upon sending the form, the user will receive from the system a confirmation message of the transmission. This message will also include a numerical code which should be retained by the user, as an element to which to refer in future for the request just sent.

Requests will be processed and the responses will be communicated to the email address indicated by the user in the specific field on the form at the latest within one month from sending the request. If the company to which the request is sent cannot pursue the same, the response will contain any reasons that prevent it from providing the information or performing the requested operation.

Please indicate in the field "description of request" on the form for exercising the rights, any useful information to clarify the scope of application of the request and its motivations.

By "sending" the request, the Interested Party acknowledges that the Lutech Group company to which he addresses the request [see mandatory field "Recipient company"] will process the data entered in the form for the purpose of "exercising the rights of the Interested Party in accordance with the GDPR". Personal data will be processed by the Authorised Personnel of Lutech Group company to which the request is addressed, in accordance with the rules and policies of the same.

Right	Description
Access to personal data – art. 15 GDPR	The Data Subject REQUESTS CONFIRMATION as to whether or not the Controller is processing personal data concerning him or her in accordance with what is indicated in the field "Description of request" of this form.
Request for rectification and/or update of data – art. 16 GDPR	The Data Subject asks the Controller to perform the RECTIFICATION and/or UPDATE of the personal data concerning him or her in accordance with what is indicated in the field "Description of request" of this form.
Request for erasure of data – art. 17, par. 1, GDPR	The Data Subject asks the Controller to perform the ERASURE of the personal data concerning him or her indicated in the field "Description of request" of this form, mandatorily indicating, under penalty of non-fulfilment of the request, the respective motivations.
Request for restriction of processing – art. 18 GDPR	The Data Subject asks the Controller to perform the RESTRICTION of the processing of personal data concerning him or her indicated in the field "Description of request" of this form, mandatorily indicating, under penalty of non-fulfilment of the request, the respective motivations: the accuracy of the personal data is disputed; the data processing is unlawful; the data subject needs the data for the verification, exercise or defence of a right during judicial proceedings; the data subject has objected to the data processing in accordance with Art. 21, par. 1 of Regulation (EU) 2016/679.
Portability of data – art. 20 GDPR	The Data Subject asks the Controller to RECEIVE the personal data concerning him or her indicated in the field "Description of request" of this form, in a structured, commonly-used and machine-readable format.
Objection to processing – art. 21, par. 1, GDPR	The Data Subject OBJECTS to the processing of his or her personal data by the Controller in accordance with Art. 6, paragraph 1, point e) or point f) of the GDPR, on grounds related to his or her particular situation indicated, mandatorily, under penalty of non-fulfilment of the request, in the field "Description of request" of this form.
Objection to processing for direct marketing purposes – art. 21, par. 2, GDPR	The Data Subject OBJECTS to the processing of his or her personal data by the Controller carried out for the purposes of sending advertising material or direct sales or for the completion of market research or commercial communication, in accordance with what is indicated in the field "Description of request" of this form.
Objection to automated decision-making process – art. 22	The Data Subject exercises the right NOT TO BE SUBJECT to a decision based solely on automated processing, including profiling.
